Privacy Policy

Remarkable Occupational Therapy (“We”) is committed to providing high quality, person-centred occupational therapy services to you and this policy outlines our ongoing obligations to you in respect of how we manage your Personal Information. We have adopted the Australian Privacy Principles (APPs) contained in the Privacy Act 1988 (Cth). These principles may be obtained from the website of The Office of the Australian Information Commissioner at the following link. 


1. What is personal information and why do we collect it?

The Privacy Act defines “personal information” as meaning information or an opinion about an identified individual or an individual who is reasonably identifiable. Examples of personal information we collect include name, date of birth, address and contact details. This is so we can easily identify you for the primary purpose of providing our services. 


2. How do we collect your personal information?

We can collect or view your personal information in many ways including:

  • Directly via interview, phone or written correspondence (i.e., email).
  • Indirectly via our website (i.e., referral or enquiry form), social media (i.e., Facebook page) or a third party involved in your care (i.e., doctor, support coordinator). 


3. Purpose of collection

We collect your personal information for the primary purpose of providing our services to you.

At times, we may also use some of your personal information (e.g., age, gender) for secondary purposes closely related to the primary purpose. This includes training and supervision of other health professionals. 

We do not collect your personal information for any purpose related to marketing, competitions or offers.


4. Sensitive Information

The Privacy Act defines “sensitive information” to include information or opinion about an individual's racial or ethnic origin, political opinions, membership of a political association, religious or philosophical beliefs, membership of a trade union or other professional body, criminal record or health information. As a health provider, we will only collect and store sensitive information pertaining to your health information. 


Your health information will be used by us only:

  • For the primary purpose for which it was obtained.
  • For a secondary purpose that is directly related to the primary purpose.
  • With your consent, or where required or authorised by law.


5. Third Parties

Where reasonable and practicable to do so, we will collect your personal information only from you. However, in some circumstances we may be provided with information by third parties, including your doctor, NDIS support coordinator and / or a family member. In such a case we will take reasonable steps to ensure that you are made aware of the information provided to us by the third party.


6. Disclosure of Personal and Sensitive Information

There may be several circumstances in which we disclose your personal and / or sensitive information related to the primary purpose in which it was obtained. This includes:

  • Third parties (e.g., your support person, other health professionals), but only to whom you consent; 
  • Where required or authorised by law; and
  • Where it is reasonably necessary to prevent or lessen a serious threat to the life, health or safety of a person or group of people.

The information we disclose may be in the following forms:

  • Verbal (e.g., in-person, via phone or Telehealth); and
  • Written (e.g., emails, reports or letters). 


7. Security of Personal and Sensitive Information

Both your personal and sensitive information is stored in a manner that reasonably protects it from misuse and loss and from unauthorised access, modification or disclosure. We collect and/or store your information using several third-party websites and applications. You can view their security policies below:

  • Website – GoDaddy (protected by reCAPTCHA and the Google Privacy Policy and Terms of Service apply).
  • Clinical Management Software – Cliniko (we utilise two-factor authentication when using this application). 
  • Electronic Form Builder – SnapForms.


Should we conduct standardised clinical assessments that require the use of an online scoring system (Q-Global, by Pearson), we are obliged to notify you that we have opted into General Data Research. This means we are permitting Pearson to:

  • De-identify examinee (you) data from our account, sub-accounts, and users, meaning they will remove all personally identifiable information, making it unidentifiable to you.
  • Use the de-identified data to improve the quality of their professional tools and services. 
  • Improve the experience of Q-global users. 


When your information is no longer required for the primary purpose for which it was obtained, we will take reasonable steps to destroy or permanently de-identify it. However, most of the information is or will be stored in client files which will be kept by us:

  • For an adult – a minimum of 7 years.
  • For a child – until they reach an age of 25.


8. Access to your Personal and Sensitive Information

You may access the information we hold about you and to update and/or correct it, subject to certain exceptions. If you wish to access your information, please contact us in writing. We may require identification from you before releasing the requested information. We will not charge any administrative fee to provide a copy of your personal or sensitive information.


9. Maintaining the Quality of your Personal Information

It is an important to us that your personal information is up to date. We will take reasonable steps to make sure that your personal information is accurate, complete and up-to-date. If you find that the information we have is not up to date or is inaccurate, please advise us as soon as practicable so we can update our records and ensure we can continue to provide high quality services to you.


10. Privacy Policy Updates

This Privacy Policy may change from time to time in line with the APPs or changes in our business practices.
