Yes, but it doesn't need to be completed by a health professional (e.g. GP). You can complete the referral form yourself or on behalf of a family member, or you can request your support coordinator to complete it. There are some cases where it is preferred to come from a health professional, such as for a driving assessment.
Our NDIS prices are set in line with the current NDIS pricing arrangements. We believe this rate is reasonable based on a number of factors, including our experience, our high-quality of service (meaning you are getting value for money), and the high demand for our services.
Please note that this rate still remains negotiable pending your available budget. We may reduce our rates for NDIS participants on a case-by-case basis.

We are completely mobile, meaning we provide services in the place you likely feel most comfortable - your home. If for any reason your home is unsuitable for an assessment, an alternate location can be arranged. Current alternate options include:
Depending on your therapy goals, a session in your community may also be helpful and beneficial to you, and plans for this can be discussed throughout engagement.
We can travel anywhere between Palm Cove and Gordonvale up to five days per week. We also travel to Innisfail on a fortnightly basis on a Wednesday.
As of 2024, we are no longer travelling to the Tablelands or Port Douglas/Mossman regions.
Telehealth may be an alternative option if deemed appropriate for the service you require.
If you need to cancel an appointment it is recommended you (or a support person) contact us by phone or email before 4pm the day before your appointment to avoid a cancellation fee. If you contact us after this time or on the day of your appointment there will be a cancellation fee of 85% added to your invoice. Should we arrive at your scheduled appointment at the agreed location (e.g. your home) and no one is present without prior notice, the appointment (including travel) will be charged at 100% of the scheduled fee for that session.
If we need to cancel the appointment due to operational reasons or illness of the clinician, the appointment will be rescheduled at the earliest convenience at no financial penalty to you.

We understand that we cannot service every referral or enquiry we receive, for many different reasons. This includes the skill set required to support your referral request and disability needs, or due to our waitlist.
